General Shotgun Tee Time Start: 8:30 a.m. for 18 holes
East Shotgun Tee Time Start: 11:00 a.m. for 9 holes
Golf Format: 18 holes at General or 9-hole option at East Course. Four person scramble, shotgun start, no handicap, fun activity, golf prizes, & blind drawings
Cost: General Course & East Course
General 18 Holes (lunch included): Non-Member $120, Eagle Ridge Member $90
East 9 Holes (lunch included): Non-Member $110, Eagle Ridge Member $80
Included: Cart, lunch, golf prizes, & donation to Galena Food Pantry
Lunch Only: $25
Lunch at Eagle Ridge Resort: Your registration includes a sit-down lunch at Eagle Ridge starting at 2:00 p.m. This includes the buffet black angus hamburger, coleslaw, baked beans, chips, & watermelon. Vegetarian burger on request.
Silent Auction: Located at Eagle Ridge Inn on the lower level on Monday, July 20 from 3-5:00 p.m., and Tuesday, July 21 from 11:00-3:00 p.m., at Eagle Ridge.
Registration is a simple two-step process. First submit the player(s) names and contact info, then complete your payment.
Note: Golf will be played in foursomes. You may turn in registration for 1, 2, 3, or 4 players. If you don’t have a complete foursome, we will make a foursome for you.
Registration: Complete your registration online here or by mailing this hard-copy form to United Churches of Galena Food Pantry, PO Box 6401, Galena, IL 61036.
Payment: Please note that payment is per person. Complete your payment via the *online process below (PayPal/major credit cards) or by making a check to United Churches of Galena Food Pantry and mailing it to the PO Box listed above.
*Please note that online payment includes a $3 surcharge per person to cover transaction fees.

Levels
Major Sponsor, $1,000+: Large name & logo printed on marketing, automatic hole sponsor sign, large banner at the event, website, and full page thank you ad; and recognized at the event. We will need an electronic copy of your logo.
Friends of Food Pantry, $500 – $999: Smaller name & logo printed on larger banner at the event, automatic hole sponsor sign, website, and full page thank you ad; and recognized at the event. We will need an electronic copy of your logo.
Tee Box Sign Sponsor, $100: Name & logo on sign, name on banner, name in full page thank you ad, and recognized at the event. We will need an electronic copy of your logo.
Hole Prizes or Silent Auction Donor (gift certificate, gift basket, etc.): Name in full page thank you ad, and recognized at the event.
Other Monetary Donation: Recognition dependent on donation; name in thank you ad.

- Payments can be made via the online process below (PayPal/major credit cards) or mailed via check (made out to Galena Food Pantry and mailed to Galena Food Pantry, PO Box 6401, Galena, IL 61036). If you are paying online, please consider covering the cost of the transaction fee when prompted, in order to maximize your donation to the Food Pantry.
